Beach Street Solution

Beach Street utilized the Case Management solution to create an electronic form to be filled out by the member of clinical staff observing or discovering the incident. Once an incident type was identified the rest of the data gathered was specific to that incident type. For example, if a bed fall occurred the user would be prompted to enter the state of the bed guards (i.e. up, fully down, half down etc.) This made filling in the forms quicker, more comprehensive, and accurate.

The completed digitally signed form is then routed via Documentum workflow to the quality control department allowing them to rapidly respond and manage the situation accordingly.

Using a combination of industry leading capture and Enterprise Content Management software and our solution frameworks we were able to rapidly and cost-effectively deliver a comprehensive Patient Incident Tracking solution.

Benefits/ROI:

The customer recognized a positive ROI on the project within twelve months. The hospital gained significant insurance as a result of their ability to better defend lawsuits with complete accurate documentation and records. Serious Incidents were addressed within an hour, all incidents within 24 hours. The project was so successful that the solution approach was applied to other areas such as smoking cessation where a rapid response was required.

This teaching hospital is regarded as one of the top medical centers in the United States. It has a prominent research center and staffs nearby top-notch universities.

Problem:
This large hospital had invested substantially in Documentum and Captiva to digitize their paper medical records and forms. They had achieved great efficiencies in their timeliness of access to medical records and had improved clinical care as a result. As a next step they wanted to tackle an area that has great impact on how a hospital is viewed from a malpractice insurance perspective, namely Patient Incident Tracking. Patient Incidents are events such as falls, accidents and other incidents that can drive a plethora of lawsuits. Key to dealing successfully with these incidents are timely and accurate responses and records.
